If you have been off for more than four days in a row, but less than seven, you can self-certify your illness using a SC2 form. The Self-Certification Form (SC2) will cover your first 7 days off work and can be obtained from your employer, downloaded from the www.gov.uk website, and are also available to collect from the display unit in the surgery foyer.
If you require a Doctors medical certificate, you will need to ring Reception after your 7 day self certification period and book a routine advice call with the GP. The medical certificate will be sent to you via SMS messaging when approved.
From July 2022 legislation changed and fit notes no longer need to be signed. They can be issued by doctors, nurses, pharmacists, physiotherapists and occupational therapists (see link to Gov.uk)

Please note: Sick/fit notes can only be issued when they are due, they can be back dated but they cannot be future dated, emergency or urgent appointments should not be used to request sick or fit notes.
If you are off work due to a hospital admission, a sick note can be issued by the hospital to cover the period of time you were in the hospital. Once discharged, your GP can then issue a sick note based on your hospital discharge letter.
